Staff Pharmacist sought by Amrbosia Healthcare, Inc. in Palm Desert, CA.
Duties: Responsible for providing safe and appropriate pharmacy services to Ambrosia patients in accordance with the policies and procedures of Ambrosia, pharmacy practice professional standards, and applicable regulatory entities. Coordinate patient services with the Nursing agencies, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Collect and organize all patient-specific information needed to determine the suitability of patients for home care. Design, recommend, monitor and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients. Modify the pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient-specific information. Provides medication-use education to Ambrosia staff, other professionals, home care patients and/or their caregivers. Prepare and dispense medication using appropriate techniques and following Ambrosia policies and procedures. Comply with state/federal laws and USP standards. Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients. Monitor and report discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion. Review and implement effective budgets and accounting to maintain daily operations. Manage and organize the ongoing functions of Ambrosia healthcare to ensure that day to day operations run smoothly.
Requirements: Doctor of Pharmacy degree or equivalent, plus one (1) year of pharmacist, pharmacy intern or related experience evaluating medication regimens; interpreting patient medical management plans; counselling patients and/or their caregivers on appropriate medication administration; employing techniques of pharmaceutical calculations to prepare intravenous products (IV admixtures, IV antibiotics, electrolyte solutions, etc.); performing quality control evaluations of medication; and ensuring compliance to pharmacy related laws and regulations and standards relative to pharmacy practice. Must have valid CA pharmacist licensure or eligibility for licensure.
